LPN (Licensed Practical Nurse) Long Term Care

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. Responsibilities:
- Administer medications and treatments as prescribed by physicians
- Monitor and record patient vital signs
- Assist with patient assessments and develop care plans
- Provide wound care and dressings
- Conduct phlebotomy procedures
- Perform tube feedings and assist with feeding patients
- Assist with pain management techniques
- Provide education and support to patients and their families
- Collaborate with healthcare professionals to ensure coordinated patient care
- Maintain accurate and up-to-date medical records
Skills:
- Proficient in ICD-10 coding for accurate documentation and billing purposes
- Experience in infant, toddler, and home care settings
- Strong knowledge of pain management techniques
- Familiarity with family planning methods and counseling
- Ability to develop effective discharge plans for patients transitioning to home or other healthcare facilities
Requirements:
- Valid LPN license in state
- CPR certification
- Excellent communication and interpersonal skills
- Ability to work well within a multidisciplinary team
- Strong attention to detail and organizational skills
